Child Photo Release
Consent that the photographs, electronic images and videotapes of my child/dependent taken during The Refinement Studio LLC events may be used by The Refinement Studio LLC for public relations and marketing purposes – brochures, flyers, posters, website or Facebook. The above mentioned photographs, electronic images and videotapes will not contain child’s name, age, grade, home address or parent/guardian information. Download the Child Photo Release form.
Adult Photo Release

Medical Condition Forms
Child Declaration of Medical Condition
Declares your child has been diagnosed with a medical condition that requires medication, or physical help from an adult. Download the Child Declaration of Medical Condition form.
*In the case of such a condition, the parent or guardian is required to remain on the premises for the duration of the workshop or party.
Adult Declaration of Medical Condition
Declares you have been diagnosed with a medical condition that requires medication, and/or physical assistance. Download the Adult Declaration of Medical Condition form.
*If such a condition exists, the participant must provide the necessary medication and/or assistance. Due to insurance restrictions, The Refinement Studio LLC can only help by calling 911, if necessary.
Dress Code
Children’s Programs
- Class attire is business casual – dress, khakis/slacks, polo/ button shirt, tights/socks and shoes
- Afternoon tea dress is formal – dress, skirt with blouse, tights and shoes, or slacks, button shirt, tie, socks and shoes
- Please, no jeans, t-shirts, sweat pants, sweat shirts, hoodies, caps, sneakers or flip flops
Adult Programs
- Class attire is business casual – dress, skirt and blouse, khakis/slacks, polo/ button shirt, nylons/socks and shoes
- Dining attire is dressy business casual – dress, skirt with blouse, stockings, and shoes, or dress pants, button shirt, tie, socks, and shoes
- Please, no jeans, t-shirts, sweat pants, sweat shirts, hoodies, caps, sneakers, boots, or flip flops
